Medical History Forms are required to be completed by all students upon admission to Briar Cliff University.

To print the form, click Medical History Form, print, complete and sign the on the reverse side and turn it into Donna Schneider, RN, BSN, in the Health Office in Heelan Hall 57. A student physical is not required by the Health Office.

Health Insurance
All students carrying nine or more credit hours on the Briar Cliff University campus are required to carry health insurance. Coverage may be through the student’s parents or through insurance that students carry on their own. Policy information is requested during the validation process and is maintained as a part of the student’s University record.
